IMPORTANT: Please print and fill out the forms listed below and send them to Coach (see e-mail for address) by Saturday Feb 20th:

• Player Membership Form
• Medical Release Form (needs Notarizing!)
• S.A.G.E. Pledge Form
• A new or very recent photo (head shot 1.25" square)
• A copy of your Birth Certificate

*** Practices Every TUESDAY @ 7:30-9:00pm at Arbor School ***

*** AND Every SATURDAY @ 12-2pm at Piscataway High School turf field (weather permitting). ***
